Appointment Booking:
Call us at 604-278-3322 to book an appointment. We are available for telehealth (telephone or video) or in-person appointments.

Update us as soon as possible regarding any changes in your personal information (name, address, phone number, etc.)
Medical Coverage for BC Residents:
New Patients:
Bring a list of all current medications and any past medical records.
To ensure that we are aware of all important medical information and history, you will complete a medical questionnaire at your first appointment.
Patients under the age of 18: Bring immunization records for us to make a copy for your chart.
Set aside at least 30 minutes for the first appointment.
We do not accept new walk-in patients. Please call (604-278-0878) ahead of time to book an appointment.

Parking:
Maximum parking time on-site is 90 minutes and is only available for patients during their appointments.
Do not use the parking lot before or after appointments.
Park your car within the lines.
Handicap parking: Available *only* for those with the proper permit.
Additional Parking: There is 2-hour street parking on Lansdowne Road beside the clinic.
Patient Confidentiality:
Consent (verbal or signed) must be provided by the patient in order for us to disclose any information to relatives. Without consent, the patient must have an appointment with the doctor to review the report.

Please note the exceptional circumstances of the COVID-19 pandemic. As of April 2020, we provide the majority of our services over telephone and videoconference until further notice. Please read our COVID-19 Protocols for more information.
We are happy to clarify any pertinent questions arising from an earlier office consultation. However, as we receive over a hundred phone calls during the day, we are unable to address all questions comprehensively. We will try our best to answer your inquiries and help you find a comprehensive source of information, if applicable.
Transfer of Records:
Patients coming from another practice may be asked to have the previous medical record transferred from their previous doctors. A consent form has to be signed before any transfer can be arranged. Most, if not all doctors, charge a fee of $50-80 for processing past records. It is important that this fee is paid promptly so there is no delay in the record reaching our office.
Urgent Medical Conditions:
If you experience any of the following symptoms, we strongly advise you to proceed to the nearest emergency room:
Severe shortness of breath
Moderate or severe chest pain, especially not relieved by nitroglycerin
Possible bone fractures
Loss of consciousness
Severe headache, especially if there is no history of headache
Loss of function/sensation of limbs, facial paralysis, sudden speech impairment or vision loss
Seizure lasting over than 5 minutes
Ingestion of poison and other chemicals
Fever in newborn less than 3-4 months of age
Severe abdominal pain
Large laceration or significant facial laceration
Uncontrolled nasal bleeding
Visits to Walk-in Clinics:
If you are to visit a walk-in clinic, please share a copy of any lab and x-ray report ordered by the walk-in physician for our own records. Keep a record of the treatment and prescription you receive from the clinic. We strongly advise you to download the walk-in encounter form and take it with you when you visit a walk-in clinic.
